On February 2, 2023, the NYC Health + Hospitals Office of Ambulatory Care and Population Health announced that Lincoln Medical Center achieved stellar performance on the Diabetes and Hypertension SMART Targets for July–December 2022, increasing its controlled rates by 3.2% and 3.6% respectively. Diabetes and hypertension are two of the most common diagnoses in our ambulatory care clinic. This accomplishment reflects the hard work of multidisciplinary teams — providers together with clerks, PCAs, nurses, clinical pharmacists, nutritionists, community health workers, and population health and disease management nurses.
