Team-based geriatric care for a large older population — polypharmacy, dementia, falls, functional independence, and rehabilitation.
The South Bronx has a very large older population, which makes Lincoln an excellent place to learn comprehensive geriatric care of complex patients. The service uses a team-based approach with its own dedicated pharmacist, psychiatrist, social worker, and physiotherapist. Polypharmacy, dementia, falls, functional independence, and rehabilitation are addressed in detail. The Geriatrics clinic is integrated with the Palliative Care service so that patients’ goals of care are properly addressed.
