Hospice and Palliative Care

An interdisciplinary team certified by The Joint Commission as an Advanced Palliative Care Program, active since 2004 and seeing more than 1,000 inpatients a year.

The Palliative Care service is an interdisciplinary team that provides supportive care to patients and families living with advanced illness. The focus is pain and symptom management, goals of care, directives, and advance care planning. Palliative care aims to prevent and relieve suffering and to support the best possible quality of life, regardless of the stage of disease or the need for other therapies, and it broadens the traditional disease model of treatment to include a person’s goals and their spiritual and social needs.

Within NYC Health + Hospitals, Lincoln is among the top providers of palliative care. Lincoln Medical Center handles more than 125,000 Emergency Department visits, nearly 500,000 ambulatory care visits, and almost 20,000 inpatient discharges each year. The palliative care consultation service has been active since 2004 and now sees more than 1,000 inpatients a year, with an additional 800+ ambulatory visits. The program is certified by The Joint Commission as an Advanced Palliative Care Program and sponsors an ACGME-accredited Hospice and Palliative Medicine Fellowship.
